Patient-centered care is at the heart of care provided by primary care providers. As patient-centered models and PCMH recognition becomes more main stream, JSI is at the forefront of assisting providers to transform to these models.
JSI has assisted many provider practices in achieving Patient Centered Medical Home (PCMH) designation through the National Committee for Quality Assurance (NCQA), the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC) and the Joint Commission (TJC), advancing their recognition level, or updating their recognition to more current standards. JSI also recognizes the value in health centers working toward the new PCMH Distinction in Behavioral Health Integration recognition from NCQA and has extensive experience in assisting with behavioral health integration in primary care practices.
The Texas Association of Community Health Centers (TACHC) has contracted with JSI to provide PCMH documentation review and transformation coaching for community health centers throughout Texas and develop an eLearning PCMH Resource Library.
