Advancing Team-based Care through Technical Assistance
The Colorado Health Foundation launched this funding opportunity for safety-net clinics throughout Colorado on February 15, 2020. CHF contracts with JSI to provide TA related to their transformation to a team based care model. For the financial TA component, Non-Profit Finance Fund (NFF) partners with JSI on this initiative. Think Equity was the third partner added in 2022 to provide TA related to equity initiatives. All partner teams will work seamlessly to support practices to embed TBC and ensure it is sustainable financially, culturally, and operationally. All partner teams have been working with safety-net clinics to provide coaching, strategic planning, regional collaboratives, and in-depth work with their regional accountable entities (RAE) to support enhanced payments after the two-year period. In addition, the Foundation funds the clinics up to $125,000 to support clinic staff participation in the funding opportunity and any education and travel-related expenses associated with implementing team-based care.
CO Team Based Care Initiative
Assisted a cohort of primary care practices (nonprofit and private safety net providers) to optimize existing care teams to deliver coordinated, high quality care by working collaboratively with patients to accomplish shared goals

District of Columbia Health Systems Plan
Worked with various groups in the District of Columbia (DC) to develop a Health Systems Plan (HSP) that serves as a guide for the development of a comprehensive, accessible, and equitable health care system that provides high quality, cost effective care for DC residents.

Lahey Health Community Health Needs Assessment
Conducted a comprehensive Community Health Needs Assessment (CHNA) for the communities served by the Lahey Health System that manages hospitals, physicians and other health services in northeastern Massachusetts.

Center for Care Innovations (CCI) Population Health Learning Network Evaluation
Evaluated the success of CCI’s Population Health Learning Network in bringing together safety net primary care organizations to strengthen and advance their population health management strategies
