CO Team Based Care Initiative
JSI with external partners, Leibig-Shepherd, LLC, Kaiser Foundation Research Institute, and the Colorado Community Health Network, serves as the technical assistance partner (TAP) for The Colorado Health Foundation’s Team Based Care initiative. This goal of this work is to improve clinical outcomes, provide better access to care, improve support for complex patients, and reduce provider and staff burnout.
Lahey Health Community Health Needs Assessment
Conducted a comprehensive Community Health Needs Assessment (CHNA) for the communities served by the Lahey Health System that manages hospitals, physicians and other health services in northeastern Massachusetts.
Advancing Team-based Care through Technical Assistance
The Colorado Health Foundation launched this funding opportunity for safety-net clinics throughout Colorado on February 15, 2020. They contract with JSI, Nonprofit Finance Fund (NFF), and Think Equity (TE). All partner teams work seamlessly to support practices to embed TBC and ensure it is sustainable from the financial and equity lenses.
Health Information Technology, Evaluation and Quality (HITEQ) Center
The HITEQ Center is a HRSA BPHC funded National Training and TA Partner (NTTAP) that supports health centers in using health IT to be data-driven organizations that provide comprehensive, high quality, and well-documented care that addresses barriers and realizes value.
CO Team Based Care Emerging Leaders Program
To accelerate the development of talented primary care staff to positively impact their practice organizations and serve as spokespeople and exemplars for others in similar roles, The Colorado Health Foundation funded JSI to provide leadership development in primary care to 23 patient facing staff and providers at practices implementing team based care.

For 40 years, we have delivered innovative solutions to client problems. Headquartered in Boston, we’ve worked in nearly every community in Massachusetts and manage at least 150 health care and community health projects in the U.S. at any given time.
