Case Studies of Addressing Social Determinants of Health

This 2018 AcademyHealth issue brief introduces four examples of state and local linkage of payment reform to addressing one or more social determinants of health. The cases include Burlington, VT (for homeless persons); Muskegon, MI; Cincinnati, OH (for pregnant women and infants); and Greenville, SC (for persons without health insurance). Comparing the sites, the brief considers factors that support innovation, lessons learned, and proposed strategic areas for further development.
